Position: Occupational Therapist (OT)
Shift: Day Shift (08:00 AM – 04:30 PM)
Shift Length: 8 hours
Contract Duration: 13 weeks
The Occupational Therapist will assess, plan, and deliver rehabilitative services to residents in a skilled nursing setting. The clinician will help patients regain independence, improve daily living skills, and enhance overall functional abilities through personalized, evidence-based therapy plans.
Key ResponsibilitiesPerform comprehensive evaluations to determine patient functional status and therapy needs.
Develop individualized treatment plans targeting ADLs, functional mobility, strength, coordination, cognition, and safety.
Provide skilled interventions such as therapeutic activities, upper extremity strengthening, adaptive equipment training, and compensatory strategy education.
Document evaluations, daily treatment notes, progress updates, and discharge summaries accurately and promptly.
Collaborate with COTAs, PTs, SLPs, nursing staff, and physicians to support coordinated care.
Educate patients and families on home programs, safety techniques, adaptive strategies, and equipment use.
Ensure compliance with facility policies, state regulations, and professional practice standards.
Participate in care planning, interdisciplinary team meetings, and quality improvement initiatives.
Active state licensure as an Occupational Therapist (OT).
Experience in skilled nursing or long-term care preferred.
Strong communication, assessment, and documentation skills.
Ability to work independently and manage a varied caseload.
