The UAS Assessment RN independently performs comprehensive home visits to assess functional, psychosocial, and environmental safety needs for diverse patients, establishing individualized care plans to support safe home living. The role requires strong communication skills, cultural competence with bilingual abilities preferred, and knowledge of Medicare, Medicaid, and relevant healthcare regulations. This position supports interdisciplinary teamwork with minimal supervision, focusing on community-centered long-term care for diverse populations across multiple New York regions.

Your talents for listening and communicating with a culturally diverse population that includes care givers as well as other providers along with your time management skills will be key to your success.
Multiple openings for the following regions: Westchester, Bronx (Spanish speaking preferred), Queens (Mandarin or Cantonese speaking preferred ), Brooklyn and Manhattan (Spanish, Mandarin or Cantonese speaking preferred ). We also have openings in our upstate counties such as Rockland, Putnam, Sullivan, Ulster, Orange, and Dutchess.
