Job Description

TITLE: Field Nurse Supervisor

HOURS : M-F Days Every other weekend  

SUMMARY:  working 2 days in the office:94 W 225th St, 10468(The Bronx); 3 days field
responsible for the direction, coordination and direct supervision of the agency’s paraprofessional staff(HHA)
when they are in the office, they will be conducting orientation or providing HHA instruction (material provided) and following up on care plans over the phone, when they are working in the field they will be doing HHA supervision and revision of care plans, they will work in clustered areas in the field, they are not expected to go to areas that are not near each other in the same day.

DUTIES:
Assess patient at start of care to develop a plan of care

Initiates Plan-of-Care and makes necessary changes as patient's needs evolve

Evaluates patients in their homes

Instructs and advise patients and/or families as well as the home health aides in their care

Provide in-field professional supervision to home health aides, personal care aides, companions, housekeepers, and nurses.

Reassess patients every six months to determine any condition change as well as supervise the home health aide to ensure proper care

Conduct on-the-job training for direct care staff to ensure high-quality care delivery.

Support continuous professional development of home care personnel through mentoring and guidance.

Oversee and evaluate the performance of care staff to ensure adherence to best practices and organizational standards.

assists in classroom and field competency testing.

MIN QUALIFICATION:  RN, BSN, 1 yr, LHCSA, or CHHA

QUALIFICATIONS: 
Current New York State License as a Registered Professional Nurse.
Baccalaureate Degree in Nursing or a related health or human services field preferred.

Minimum of one year of nursing experience in an acute care facility.

One year of satisfactory nursing experience in Home Care preferred.

Thorough knowledge of the scope of practice for both professional and paraprofessional healthcare.

Strong ability to read and interpret safety rules, operating instructions, and procedure manuals.

Effective communication skills for interacting with clients and employees.

Competence in performing mathematical operations related to medication calculations and measurements.

Excellent organizational, writing, and interpersonal skills
