We are seeking Nurses who believe in our mission to help patients live out their best lives at home. That means coordinating care and making sure to deliver on quality care. That care is provided within the Agencys scope of services, policies and procedures, state and federal laws and regulations, the Nurse Practice Act and Accreditation entity requirements.

We are Currently hiring for FULL TIME, PART TIME & PRN.

To be considered for this position the following must be listed among minimum qualifications:

• Graduate of an accredited school of professional nursing
• Home Care Experience preferred.
• Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
• Communicates effectively and in accordance with Agency requirements.
• Assess the continuing needs of the patient. Performs initial and ongoing patients assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services, as needed.
• Participates with team of RNs, LPNs and or Home Health Aides to provide effective and quality home care services.
• Promotes personal safety and a safe environment for patients and coworkers.
• Performs job duties in accordance with Agency policies, procedures, and professional and community standards.
